any errors? systemctl status clamd@scan

Try removing and reinstalling?

All I can think of right now to try before you nuke and reinstall the OS , worth a try

are you running SELinux?
GetEnforce either 1 enforcing or 0 Permissive.

clamav-freshclam-1.4.3-3.el10_2.x86_64

or try dnf remove clamav*

ok bare metal, rules out vps packaging.
You can try (after removing) dnf install clamav*"

Trying to help but hard to say why, I mean can be SELinux, how you set permissions. Many variables here
