When I visit some of my pages I see an error:
No input file specified.
I am not sure why?
I should also add that if I don’t add /index.php I often get redirected to LOCALHOST. 
After I manually add /index.php it’s almost like it’s cached and then I can do example.com/test/ without the index.php and it loads fine.
