The database hostname will be ‘localhost’ or the hostname of the Virtualmin server, depending on where the database server is accessed.
In Virtualmin → Edit Databases you can find the login name and password to use.
The best way to use subdomains is to create a new virtual server for each domain / subdomain. You should do that.
The community will be able to be more specific in the assistance that it can offer if you specify the version of Webmin and Virtualmin that you use and the log of the errors that you get.
